Liability Release

When you sell a dog that has had bitework training, be it sport or pp oriented, do any of you use a liability release form? I'm thinking maybe I should start using one. I know they won't keep me out of court, but it would be nice to have something to show a judge/jury that states the new owner of the dog understands the liability associated with owning a dog that is trained for bitework, and assumes all responsibility for the dog and it's actions. At least then I can show that the new owner was aware of the risks involved, if I end up in a situation where the dog bites someone and they try to play dumb and pretend they had no idea that could happen.

Heck, for that matter it might be good to just have one when I sell any dog, trained or not.

Anyone willing to post the form that they use?
